Those needing coverage are encouraged to first contact their local agent. Individuals may contact the Facility directly to apply for coverage. WHO MAY FILE APPLICATION Application may be filed directly by any person or legal entity having an insurable interest in the property. Application may be filed on behalf of the applicant by a Producer licensed by the Department of Insurance and Securities Regulation of the District of Columbia. NEW BUSINESS APPLICATION • The application form consists of multiple pages. Two (2) completed and signed copies must be submitted to the Facility. One copy is to be retained by the producer or applicant. Upon receipt by the Facility application forms are reviewed for proper completion: If application is being made directly, please review the Customer Information Sheet for more information The following forms are available to download and print locally.
